Is the balance shaft noise just an iritating noise, or is it something that could be harmful to the engine?

IMO, it's main function is to irritate the driver.
Of course any vibration or rattle does not promote longevity, but I don't think that this will cause any damage.
The gears are the only place that could see any effects from the rattle. Although the vibration may somewhat transfer to other components (timing chain, camshaft, etc.) I would think that the effects would be minimal if any at all.
Remember this is Just My Opinion, others may feel that it is a real problem.
My truck has little if no rattle. Evidently the clearances must differ greatly in different engines , since Mello's was much more noticeable than mine.
