I just switched wheels to 4.5" bs with 33x10.5 tires and cut out flares. When I went in to have the new tires mounted on my new wheels I was informed that my pass. side ball joint and idler arm are in need of replacing but that wear and tear was with stock wheels and 30x9.5 tires.

15x8 Cragar Rock Crawler Wheels w/ 4" of backspacing
31x10.5x15 BFG TAKO's
No spacers up front, 2" in rear

No balljoint/CV problems since I went to the new wheels/tires. The end of January will make one year since I got the wheels put on.

The roads I travel aren't perfect, but they're by no means horrible, and I don't do a ton (not every weekend) of offroading, and I usually airdown when I do.

1.25" spacers up front, 3" on the rear.
Stock wheels (ZR5) w/6" backspacing
yes to cutout flares
33" tires
With 31" tires the lower ball joints went bad, replaced both lower under warranty. Both wheel bearings replaced. One under warranty and one just after the 33's.

Switched to:
15x8 Centerline's with 5.5" BS
30x9.5's
No front spacers, and 1.5" spacers in back
No fender trimming--Yet, but probably summertime.
Idler arm is going bad, but most likely not due to wheel switch. Some tire rub in turns when front squats, but nothing extreme (the reason for 2" BL and fender trim this summer and increase tire size).

15x8's with 4" BS, 31x10.5" tires.

On bad roads I was replacing balljoints and wheel bearings once a year (~20k a year), never had a problem with CV's though.

Can't comment on clearance problems though as mine is a first gen S-10 and it would not really relate to your truck.
