I changed out my hvac knobs 4 the bravada style.

I have the same leer cap w/a dome light.
Mines not hooked up either.

Mine is made by leer, it was on the truck when I bought it.
The back end is more like a 4-door blazer vs. A 2-door.
Locally there is 1 for sale in ri that is made by jeraco, also a 4-door style.

According to lmc: the rears are zr5 only
The fronts are listed as being the same between the zr5 & the extreme models.
Might make it a little easier for you by looking @ more than 1 truck style.
